Informed Consent Form

Spinner Software™ is seeking dreams to be used in the norming process for the software package "DreamSpinner™" Please fill out the dream research User Registration Form completely. Your anonymity will be preserved. We are interested in the word frequency count found in the dreams and the basic demographics we asked for in the registration form. Portions of your dreams may be quoted in the write up of the norms but will not be associated with a name or specific personal information other than the general demographic information given in the registration form. It is your responsibility to change the names of people and places in your dreams to protect anyone you drempt about.

Your participation in this norming process is entirely voluntary. You will be given a subject number after you have filled out the Consent Form sent to you by snail mail so I can have your signature. That way your name will not be used in any way through out the norming process. It is necessary to fill out the address information in the User Registration Form for me to send you the official Consent Form.

I understand my participation is entirely voluntary. I understand, also, that my anonymity will be preserved. I will be given a Subject Number for identity. I understand portions of my dreams may be quoted in written material about the norming process and that information will not give any names or personal information except any basic demographic information provided by the subject in the User Registration Form. I agree to change the names and places in my dreams when I record them and post them to the Spinner Software™ website to protect anyone mentioned in my dreams.
